Solution
Controller blades are numbered bottom to top; the bottom blade is slot 1 and the top blade is Slot 5.

Slot 5 = 8a

Slot 4 = 88

Slot 3 = 86

Slot 2 = 84

Slot 1 = 82

For example, if you want to reboot the MC5000 Blade that is in Slot 5, enter this command in the shelf manager:
