Steps by steps:
_ Out of 2 Tokens, even if one has status of Error you need to delete the existing token as well to fix the other token.
_ In order to delete the token, you must ensure to un-assign / revoke it from the user to who its assigned to.
_ Once done, select both the tokens and delete them.
_ At this point you will see "Downloads Available" button / option in the same FortiToken pages. On clicking it, you will get both the tokens and then follow the procedure to assign and use it.
_ Provision Timed Out status appears only if the User whom the Token was assigned , failed to activate the token within the stipulated time of activation code sent that expires. To fix it un-assign the token and re-assign and send activation code again. (This doesn't require to follow above steps)
