Solution
Important notes.
The license file is already present, otherwise check with the Fortinet local partner or local Fortinet sales team:
1) Login to the Fortigate from GUI, access: User & Authentication -> FortiToken -> Create new -> Mobile Token.
2) The activation code is from the license file.
Key in the activation code stated from the above screen-shot.
Take note that 'Serial Number' is not the activation code.
3) Once the activation code is successful, the Fortitoken Mobile will appear under GUI.
